Apply for an Individual Athlete PLAY SPORTS GRANT

The first step to apply for an individual athlete grant is to fill out this form. Your sports organization will notify you of approval. We do not review incomplete applications, nor do we notify applicants if form is incomplete (carefully, check your information before submitting).

Qualification criteria for individual grants: Local Hampton Roads Area’s parents or guardians that need required funds to pay for youth sports. BH4NC reserves the right to make exceptions to eligibility guidelines for reasonable extenuating circumstances.
